Clinical Lab Registration Requirements
All providers performing laboratory testing are required to register with the Clinical Laboratory Improvement Amendments (CLIA) program. Unless you are registered in the CLIA program, Medicare cannot pay claims for any laboratory services you perform.
If you perform clinical laboratory testing and have not registered with the CLIA program, please contact the state in which you practice:

You will be informed on how to register with the CLIA program and what fee you are required to pay.
NOTE: Performing laboratory tests without CLIA approval is a violation of the Clinical Laboratory Improvement Amendments of 1988, which may be punishable by fines and imprisonment. To avoid the denial of your claims, promptly request a laboratory questionnaire (CMS 209) from the above address.
