Electronic Signatures: What You Need to Know to Prevent Errors
When signing medical records electronically, the electronic signature MUST appear on the records when printed or your claim will be in error. CGS is still seeing an increase in errors due to inadequate provider signatures on documentation submitted for medical review. One common error we see is that although the physician signs off on his/her notes in the electronic medical record, when printed the signature does not appear on the printed copy which causes an error. This guideline can be found in the Program Integrity Manual (Pub 100-8, Ch 3, Sec 3.3.2.4).
