Checklist for Home Office Cost Statement Submission

The following checklist is intended to help a provider ensure all required items are submitted with a Home Office Cost Statement (HOCS). Please ensure the following items are included with the HOCS submission:

  • A complete hard copy of the Medicare HOCS with an original signature of the Administrator or Officer.
  • A copy of the working trial balance to support expenses.
  • Documentation to support reclassification entries, adjustment entries, and related party entries.
