Beneficiary Information Authentication Fact Sheet
When you use the myCGS DME Web Portal or call our JB Customer Support line or JB Interactive Voice Response (IVR) System, you must give specific information to access beneficiary details. This includes:
- Provider National Provider Identifier (NPI)
- Provider Transaction Access Number (PTAN)
- Last 5 digits of Tax Identification Number (TIN)
- Medicare Beneficiary Identifier (MBI)
- Beneficiary's first and last name
- Beneficiary's date of birth
- Date of service (for claim information)
Without the right information, you might not get what you need. We understand this can be frustrating, so we made this fact sheet to explain why we need this information:
- Protect Beneficiary Information: Share personal details only with authorized individuals. We can only release protected health information (PHI) or personally identifiable information (PII) if you need it to treat a patient, bill correctly, or get paid for items or services provided. Some examples of PHI and PII include the MBI, the beneficiary's first and last name, address, and date of birth.
- Prevent Fraud: Reduce the risk of identity theft and fraud.
- Ensure Accuracy: Make sure information matches our records to give correct details.
