Important Message About DDE & PPTN Access
The 2025 Annual DDE PPTN Recertification process took place June 1 – December 31.
Beginning on Monday, January 12, 2026, and over the next 6 weeks, CGS will deactivate Direct Data Entry (DDE) and Professional Provider Telecommunications Network (PPTN) IDs assigned to users who didn't submit a recertification form.
- If you didn't recertify but still have access, complete and submit the Annual DDE PPTN Recertification Form
. - If you no longer have access, complete and submit the online J15 DDE PPTN Application/Reactivation form.
