Important Message About DDE & PPTN Access
The 2024 Annual DDE PPTN Recertification process took place between June 1 – December 31, 2024.
CGS is currently deactivating Direct Data Entry (DDE) and Professional Provider Telecommunications Network (PPTN) users who didn't submit a recertification form.
- If you didn't recertify but still have access, complete and submit the Annual DDE PPTN Recertification Form

- If you no longer have access, complete and submit the online J15 DDE PPTN Application/Reactivation Form.
