Reference Guide to Avoid Duplicate Documentation for a CERT ADR
Purpose
To help providers and offices respond correctly to CGS requests for Comprehensive Error Rate Testing (CERT) Additional Documentation Requests (ADRs) (i.e., avoid sending irrelevant or previously submitted records).
Why This Matters
Sending entire charts or previously submitted documentation packets instead of the specific documentation requested can:
- Delay claim review and payment.
- Result in missing documentation.
- Not allow enough time for appeals.
What Not to Send
- Entire EMR printouts
- Duplicate submissions from prior ADRs
- Unrelated chart notes
- Full chart packets when only specific items are requested
What To Send
- The documentation listed in the ADR
- Signature pages, order/referral forms, progress notes specific to the necessary ADR
- Any test results, lab reports, or other supporting documentation that coincides with the specific documentation requested
Best Practices
- Review the ADR carefully.
- Pull the required documentation only.
- Label documents clearly.
- Check before sending.
