CMS-855A: Transplant Program & Organ Type(s)
If a hospital selects Transplant Program as a subgroup/unit on the CMS-855A provider enrollment application, an Organ Type field is also required. PECOS then collects the Transplant Program indicator and Organ Type information.
On April 6, 2026, Change Request (CR) 14262
implements system changes for PECOS to send the Transplant Program indicator and Organ Type information to FISS to process claims.
For claims to process correctly, it’s important to complete the enrollment application as follows:
PECOS Web Submissions
- Under Provider Type Questionnaire, click the “Hospital – Transplant program (Identify organ type(s))” checkbox.
- Select an organ type from the drop-down menu and enter the effective date.
- If the specific organ type options available don’t apply, select “Other Organ Type”, and enter a description in the free form text field. Do NOT enter any of the drop-down menu options in the free form text field.
855A Paper Submissions
- Under Section 2A2, mark the “Hospital – Transplant program (Identify organ type(s))” checkbox.
- In the free form text field, identify one organ type:
- Adult heart/lung
- Adult heart only
- Adult intestine/multivisceral
- Adult kidney only
- Adult liver
- Adult lung only
- Adult pancreas
- Pediatric heart/lung
- Pediatric heart only
- Pediatric intestine/multivisceral
- Pediatric kidney only
- Pediatric liver
- Pediatric lung only
- Pediatric pancreas
- Heart assist registry
- If the most common organ types listed above don’t apply, write “Other organ type” and a description in the free form text field.
NOTE: Each hospital NPI/CCN combination should have only one occurrence for an organ type.
