Misrouted Appeal Requests
CGS Part B Appeals continues to receive misrouted appeal requests from providers. In some cases, providers are using the correct contractor appeal form; however, mailing to the incorrect contractor address.
Sending your appeal request to the incorrect contractor increases your time frame for receiving a response, thus reducing the time you have to continue through the appeals process. This is also a cost to the Medicare Trust Fund.
Before sending appeal requests, please review your Medicare Remittance Advice (RA). The top left-hand corner identifies the correct contractor and the correct mailing address.
If you are appealing at the second level (Reconsideration) please refer to the page titled, "Important Information About Your Appeals Letter." This page has the address to send your Reconsideration requests to the Qualified Independent Contractor (QIC). Reconsiderations are not processed by CGS, as we are your first level Redetermination contractor.
