When you call, the Home Health and Hospice Provider Contact Center, and select an Option 1-4, the Computer Telephony Integration (CTI) system will prompt you for information in order to validate your provider and beneficiary information. Be prepared and have the following information available before you call. Option 9 is only used for general questions not related to claim specific topics.

  • Your facilities National Provider Identifier (NPI); Provider Transaction Access Number (PTAN); the last 5 digits of the Provider Tax Identification Number (TIN)
  • Beneficiary’s Medicare health insurance claim (HIC) number; the beneficiary’s first name, last name, and date of birth.
    • Option 1 to reach the Home Health and Hospice Customer Service Representatives (CSR) for assistance with home health and hospice billing, claims, eligibility, and payment.
    • Option 2 to reach an Electronic Data Interchange (EDI) Customer Service Representative for assistance with EDI enrollment, connectivity, PC-Ace Pro32, trading partners, and myCGS. 7:00 a.m. – 4:00 p.m. (CT)
    • Option 3 to reach the Provider Enrollment (PE) department that can assist with inquiries about enrollment applications, enrollment revalidation, or electronic funds transfer enrollment. 8:00 a.m. – 4:30 p.m. CT
    • Option 4 to reach the Overpayment Recovery (OPR) department for assistance with overpayment issues or questions about Credit Balance withholding. 8:00 a.m. – 4:30 p.m. CT
    • Option 5 to reach a CSR for assistance related to the pre-claim review demonstration for home health services. NOTE: Once you select Option 5, you can then choose between Option 1 for claim specific PCR questions or Option 2 for general PCR questions.
    • Option 9 to reach a Home Health and Hospice CSR for general questions. Refer to the “General or Claim Specific Questions When Calling the Provider Contact Center” article to determine if your question is considered a general question.

Please note that if you have or will be submitting an adjustment, please send the UB-04 along with the CMS-838 form.
  • If you are issuing a refund check for a credit balance:
    Send the CMS-838 and a copy of the refund check using one of the options listed above.
Send the refund check with a copy of the CMS-838 or documentation that indicates the check is for a credit balance, the quarter end date, and provider number associated with the check to the following address:
