CGS is responsible for providing you with information concerning enrollment into the Medicare program, processing Medicare claims, and communicating changes in the Medicare guidelines.
The processing of an eligible Medicare provider/supplier application requires extensive research to ensure all information provided is accurate and all appropriate attachments are supplied. Please note that there is key information that is critical to the enrollment process including but not limited to the completion of the most current CMS-855 enrollment application(s), Authorization Agreement for Electronic Funds Transfer (CMS-588), clinical lab registration requirements, and other important Medicare enrollment information.
