Diagnosis Codes Not Required on Notice of Termination/Revocation (NOTR)
CGS has received clarification from the Centers for Medicare & Medicaid Services (CMS) that a diagnosis code is not required on the Notice of Termination/Revocation (NOTR). However, providers should be aware that if a diagnosis code is reported in the DIAG CODE field on Page 03 of the Fiscal Intermediary Standard System (FISS) on a 81B or 82B type of bill (TOB), FISS will check to ensure the ICD-9 or ICD-10 code is valid.
CGS educational materials currently instruct hospice providers to submit the diagnosis code; however, updates to reflect this clarification will be made to those materials soon.
